Note that in addition to the abnormal cells themselves, there is flattening of cells at the edge of the cluster, lack of columnar shape and loss of polarity (central whorling). These features favor HSIL extending into endocervical glands over a glandular lesion. Explanatory Notes: The differential diagnosis of such clusters includes HSIL with gland extension and AGC/ endocervical neoplasia.
